Nomination form

Our volunteers help to raise funds in our charity shops, provide emotional and social support to older and vulnerable people, inspire and motivate local young people and play a key role in supporting or fundraising for a range of projects and activities.

Our annual volunteer awards recognises the incredible contribution that our volunteers make.

This year, awards will be made in 8 categories, with prizes and recognition for a winner and ‘highly commended’ in each category.

We are approaching colleagues, clients, support workers, mentors and any other people affected by the work of our volunteers to nominate any volunteer that they feel deserves a reward.

Every volunteer that is nominated will receive a certificate and awards will be presented to winners in each of the following categories:

  • Octavia Foundation Befriending Volunteer Award
  • Octavia Foundation Project Volunteer Award
  • Octavia Foundation Young Volunteer Award
